I)A moderator gone mad (Unlikely)
II)Someone who knows your password (Likely)
III)A sinister cracker who spent hours of time and effort with the sole intent of changing your avatar (Unlikely)

There are no such known security bugs in this version of PHPBB to make a script kiddy likely.

It is highly unlikely a moderator did it.

Tito, phpBB passwords are easily cracked by automated brute force.
